Job descriptionConduct primary care rounding for short- and long-term care patients Diagnose and manage chronic and acute medical conditions Develop and implement individualized care plans Coordinate care with hospice, palliative, and home care providers Participate in quality initiatives and case reviews Engage in community-level relationship-building and network development Estimated annual compensation : $174,004 $208,804 (productivity-based, no cap) Comprehensive benefits package including : Paid professional liability insurance with tail coverage Paid time off 401(k) retirement plan CME and leadership training Clinician wellness and referral programs Minimum 1 year of primary care experience Post-acute care experience preferred but not required Active and unrestricted New York medical license (or in process) DEA with prescriptive authority (or in process) Located along the southern shore of Lake Ontario in western New York Offers a rich arts and culture scene, historic landmarks, and vibrant neighborhoods Home to 17 public parks and 764 acres of green space ideal for hiking, biking, and wildlife watching Hosts numerous annual festivals and local events Affordable cost of living and excellent public and private schools Quick access to Finger Lakes wine country and upstate outdoor recreation

Position Highlights :
- Full-time or Part-time position in a skilled nursing facility setting
- Flexible weekday rounding schedule (4 5 days / week)
- Autonomy over daily schedule and patient care approach
- Collaborate with interdisciplinary teams and facility leadership
- Provide clinical oversight and mentorship to nurse practitioners
- Leadership development opportunities available
