The Part-Time Nurse Practitioner (NP) will conduct home health risk assessments within Scioto County, OH. The average home health risk assessment will take approximately 45 minutes, and the nature of the visit may vary, potentially including an annual health assessment or a post-discharge visit. The home health risk assessment will be conducted on Medicare Advantage plan members residing in Portsmouth, OH,and the surrounding Scioto County, OH.Scioto County, OH,includes locations like Portsmouth, Porter Township, and Clay Township.Candidates could be scheduled in any one of these locations during an individual shift.

Responsibilities :

• Conduct annual home health risk assessments for a Medicare Advantage health plan
• Evaluate prior and current medical history, medical conditions and medication history
• Assess for Gaps in Care and provide recommendations to the primary care physician for gap closure
• Provide education on medical conditions and medications
