At Telecare , we're seeking an ASW, AMFT, or APCC in San Diego who's ready to take the next step into supervision and real-world impact. You'll supervise a small team of 5-6 staff members providing rapid case management and resource coordination for older adults transitioning out of institutional settings.

Schedule & Pay

Full-Time | Monday-Friday | 8 : 00am-4 : 30pm

Starting Pay : $72,000 - $88,000 (DOE & San Diego differential)

About the Program

This community-based program provides short-term, rapid case management to older adults with serious mental illness or co-occurring conditions who are stepping down from institutional settings. Your team will help connect members to housing, medical care, benefits, and ongoing behavioral health treatment - ensuring stability and continuity of care.

You'll work within a collaborative, mobile team model , combining clinical insight and compassionate fieldwork to support some of San Diego County's most vulnerable residents.

Minimum Qualifications

Current ASW, AMFT, or APCC registration with the California BBS

Master's degree in Social Work, Counseling, Psychology, or related field

Valid driver's license and personal auto insurance (community driving required)

Supervisory experience preferred, or willingness to complete Telecare's supervisor training
