Responsibilities (which May Vary By Client)

• Utilize care plans personalized to each child
• Advocate for the childs needs through communication with the care team, physician, and families provide highest quality of life to each child. Provide the highest quality care to reduce risk of hospitalization
• Document through our electronic system from our tablet or any device you prefer
• Contribute to a culture caring through individual accountability and teamwork
