Our client is seeking a Home Health Speech Language Pathologist to provide therapeutic care to patients in their homes throughout the Uniontown area. This position focuses on improving patients' communication and swallowing abilities through individualized treatment plans while maintaining comfort and independence in a home setting.

Responsibilities include assessing, planning, and delivering speech-language pathology services, maintaining accurate documentation in the electronic medical record, and collaborating with interdisciplinary care teams to ensure continuity of patient care.

Required qualifications :

Active Speech Language Pathologist license in the State of Pennsylvania

Master's degree or higher in Speech Language Pathology

Minimum of one year of professional experience (Home Health experience preferred)

Familiarity with electronic medical records (EMR) systems

Valid driver's license, reliable transportation, and auto insurance

Travel : Approximately 20% of the workday will involve driving to and from patient locations
