Key Responsibilities
- Evaluate patients and create personalized therapy plans in the home setting
- Deliver evidence-based interventions to restore mobility and function
- Educate patients and families on exercises and safety strategies
- Collaborate with the interdisciplinary team to ensure coordinated, goal-oriented care
- Maintain accurate and timely documentation to meet quality and compliance standards
- Adapt care plans based on progress and physician feedback
- Support patients through discharge planning and wellness strategies
Requirements :
What We’re Looking For
Licensed Physical Therapist (PT) in the state of New MexicoAt least 1 year of clinical experience (home health preferred)Skilled in patient communication, documentation, and problem-solvingSelf-motivated and comfortable working independentlyProficient with electronic health records and mobile chartingPassionate about function-focused, relationship-driven careCommitted to our mission : to bring out the courage in othersWhy Join Us
