High Desert Medical Group is seeking a full-time candidate to assist with Disease Management's role as Certified Medical Assistant (CMA). This individual is responsible for assisting the pharmacists provide clinical pharmaceutical care to patients. Maintain a professional, positive and caring attitude at all times. This individual will provide the following but is not limited to : Patient Outreach with medication refills to meet the criteria for the Medication Adherence Star Measure. Job duties may include but not limited to patient education, outreach phone calls, fingerstick blood sugar, A1c or INR testing, Vaccine administration, direct patient care, verifying eligibility and scheduling DMP appointments.
Summary of Duties :
- Maintain a working knowledge of diabetes. COPD, CHF, Asthma, ESRD, CKD, Osteoarthritis, and other disease states.
- Daily phone calls to patients.
- Perform INR finger sticks, Fingerstick A1c, glucometer testing and other pertinent information (blood pressure, weight etc.).
- Support the mission, vision and values of the Pharmacy Department at HDMG; which includes following the Pharmacy Department's Policies and Procedures.
- Communicate with the pharmacist(s) for medication dosing and insulin titrations.
- Submit referrals if necessary for disease management and other specialty physicians.
- Submit lab orders for patients in the diabetes management program as necessary and communicate this information and any other information with the patients.
- Communicating with the IPA physicians / nursing teams to answer questions, to request labs, or to communicate dosing titrations.
- Communicate with in-house physicians, doctors, and nurses.
- Assist the pharmacy team on identifying patients with uncontrolled chronic disease states.
- Responsible for updating the diabetes patients' enrollment log, including but not limited to contact information, call updates, A1c, PCP information, and any necessary information.
- Participate in the clinical guidelines development, critical pathways, disease management and drug use programs as identified and under the mentorship of the Clinical Services Manager.
- Participate in and provide continuing education programs.
- Follow and adhere to HDMG's standards of attendance policy, as it is critical to maintain communication with patients, staff, and other departments, as well as submitting and maintaining patient information (referrals, calls, lab orders, etc.).
- Perform other duties as assigned.
Position hours are : Monday through Friday 8am to 5pm.
The pay range for this position at commencement of employment is expected to be between $22.00 - $29.33 / hour, non-exempt; however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus, restricted stock units, and discretionary awards in addition to a full range of medical, financial, and / or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered. High school graduate or equivalent.Graduate from an accredited Medical Assistant program.Maintain a current Certified Medical Assistant certification.Current BLS certification.Minimum of 1-year medical office or admitting experience preferred.Knowledge of basic medical terminology.Bilingual in Spanish, level 2 translator preferred.Knowledge of insurance plans preferred.Excellent written, verbal, and interpersonal communication skills.Ability to handle and prioritize tasks to meet all given deadlines and productivity goals.Ability to responsibly handle matters of a confidential nature.Ability to work in a multi-task, high-productive environment.Ability to type 20 WPM accurately, preferred.
