1+ year of CLINICAL RD experience (hospital etc..)

Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. SNF dialysis patients receive dialysis treatment onsite at the Skilled Nursing Facility in which they reside.

The RD will have a caseload of around 80 DaVita Skilled Nursing Facility Dialysis patients. The RD must be located in Virginia so that they are able to visit each site 2-3 times / month.

The SNF dialysis locations supported include :

• Guggenheimer Health & Rehab Center, 1902 Grace St, Lynchburg, VA 24504
• Highland Ridge Rehab Center, 5872 Hanks Ave, Dublin, VA 24084
• Charlotte Health and Rehab Center, 1735 Toddville Rd, Charlotte, NC 28214
