SUMMARY OF POSITION :

The Emergency Department Registered Nurse Navigator role aims to help facilitate the patient journey to ensure proper navigation throughout the health care system upon discharge disposition. The Emergency Department Registered Nurse Navigator also aims to support staff to assist in the coordination of care while the patient is transitioning through the healthcare continuum throughout the outpatient healthcare setting. The

Emergency Department Registered Nurse Navigator

will serve as the clinical resource to scheduling coordinators and assist in identifying appropriate services or providers for referrals. The

Emergency Department Registered Nurse Navigator

will recommend alternative locations of care when appropriate and identify the discharge needs and services.

MINIMUM QUALIFICATION(S) :

Currently licensed to practice as a Registered Nurse by state of employment or holds a multistate RN license through the interstate Nurse Licensure Compact.

AHA BLS before the end of the orientation period

PREFERRED QUALIFICATION(S) :

Registered Nurse with Emergency Department experience preferred
