Job Summary :

As the Inpatient Social Worker, you will :

• Provide supportive services and counseling to patients, clients, residents, and their families regarding healthcare and home care programs.
• Serve as an integral member of the interdisciplinary team to address social, emotional, and economic concerns for optimal patient functioning.
• Assist families in understanding complex medical situations and develop care plans by coordinating resources.
• Respond to suspected abuse, neglect, or violence per ethical guidelines and state laws.
• Demonstrate professionalism through active participation in care conferences and transitional rounds.
• Showcase strong critical thinking and problem-solving abilities while maintaining effective communication with patients and healthcare teams.
