Overview :

Sentara Health is seeking to hire a qualified individual to join our team as a Licensed Practical Nurse.

Position Status : Full Time, Night Shift (12 : 00 am - 8 : 30 am)

Position Location : Sentara Hospice Services - Suffolk, VA (travel within a 40 mile radius)

Pay Range : $22.39 - $37.32 / hour

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. Additional compensation may be available for this role such as shift differentials, standby / on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

Overview

The Licensed Practical Nurse (LPN) in a home health care setting is responsible for providing direct, one-on-one care to patients in their homes. This role involves monitoring patient health, administering treatments, managing medications, and offering essential support to patients with various medical needs. Collaboration with a team of healthcare professionals, including Registered Nurses (RNs) and physicians, is required to deliver high-quality, compassionate care that improves patient well-being.

Education

  • Trade School Graduate (Required)

Certification / Licensure

  • LPN License (Required) - Active in Virginia
  • BLS (Required within 90 days)
  • Experience

  • Serve as patient advocate in the prevention of illness or disease
  • Provide care and treatment for patients and clinical support and teaching for patient and family members
  • Customer service, testing, medication administration and the implementation of nursing interventions and care plans
  • Support a holistic approach to patient care
