• Provide direct patient care assistance in the patient's residence under the direction and supervision of the RN and according to the Plan of Care
• Effectively communicate with the Interdisciplinary Team to ensure patients and their families receive the alleviating care, comfort and compassion they deserve
• Perform or assist patient / family with home safety and activities including, but not limited to, light housekeeping and assistance with meals
• Appropriately document and monitor adequate nutrition intake, temperature, pulse, respiration, skin and basic elements of body functioning and changes
• Provide compassionate communication and emotional support to the patient, family and other employees
• Assure for compliance with local, state and federal laws, Medicare regulations and organizational policies and procedures
• Complete annual educational requirements
