Position Highlights :

• Care Plus Acute

Goal is to manage patient’s medical needs as they transition back from an inpatient stay to the community by having a face to face visit every week

Homebound program where a patient from the acute program can also be transitioned to if they are still unable to seek care in the community after 30 days

The salary range for this role is $104,500 to $156,000 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
