SUMMARY
Family Service League is seeking a part -time Case Manager for the Healthy Families Nassau program. The Case Manager will be responsible for engaging new referrals of prospective participants to determine eligibility for Healthy Families. The Case Manager will also conduct interviews and complete assessments with expectant parents or parents who have a child less than three months of age. The Case Manager will provide home visits to families located throughout Nassau County, but primarily in Hempstead, Uniondale, and Roosevelt. Travel is required.

QUALIFICATIONS
High school diploma or equivalent required.
Experience in working with or providing services to children and families.
An ability to establish trusting relationships.
Acceptance of individual differences.
Experience and humility to work with culturally diverse families.
Knowledge of infant and child development.
Willing to engage in building reflective capacity (e.g., capacity for introspection, communicating awareness of self in relation to others, recognizing value of supervision, etc.).
Excellent verbal and written communication skills required.
Proficient computer skills, including Microsoft office, required.
Valid and clean NYS Driver's License required.
Bilingual in Spanish or Haitian Creole preferred
