This role provides safe, competent quality care based on nursing theory and research to a designated group of patients and their significant others. Delivers patient care by incorporating the principles of Relationship Centered Care. Applies the nursing process based on theory, research, evidence-based practice, and organizational nursing standards. Assesses and evaluates patient care needs applying critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team to develop, implement, and evaluate the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care. Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease. Communicates effectively and professionally with patients, families, and all members of the interdisciplinary care team. Manages assignments within the Care Delivery Model of Modified Primary Nursing effectively. Role models the principles of Relationship Centered Care during interactions. Considers patient values, preferences, cultural diversity, and expressed needs in all aspects of care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to enhance understanding of patients' perceptions of care. Initiates service recovery for patients and family members with concerns.
