This role provides safe, competent quality care based on nursing theory and research to a designated group of patients and their significant others. Delivers patient care by incorporating the principles of Relationship Centered Care. Applies the nursing process based on theory, research, evidence-based practice, and organizational nursing standards. Assesses and evaluates patient care needs applying critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team to develop, implement, and evaluate the plan of care. Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care. Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease. Communicates effectively and professionally with patients, families, and all members of the interdisciplinary care team. Manages assignments within the Care Delivery Model of Modified Primary Nursing effectively. Role models the principles of Relationship Centered Care during interactions. Considers patient values, preferences, cultural diversity, and expressed needs in all aspects of care. Establishes and maintains therapeutic relationships with patients and families. Uses evidence-based practices to enhance understanding of patients' perceptions of care. Initiates service recovery for patients and family members with concerns. Demonstrates caring, respect, compassion, empathy, and active listening. Supports staff in using evidence-based practices to recognize patients' perceptions of care. Conducts purposeful rounding on assigned patients addressing key care aspects. Practices safe and efficient patient-centered care. Identifies and escalates barriers and opportunities for improvement in patient care. Demonstrates clinical skills and knowledge crucial to care quality and safety. Implements process improvement strategies based on institutional evidence. Engages in peer and institutional review processes. Collaborates with interdisciplinary teams on quality improvement projects and nursing research studies. Contributes to quality practices and compliance measures. Implements principles of high reliability to maintain a safe care environment. Escalates potential safety hazards or gaps in best practice as appropriate. Documents nursing practice to support quality and performance improvement initiatives. Performs charge nurse duties as assigned. Coordinates with nursing leadership to ensure adequate supplies for patient care. Escalates resource deficits to appropriate nursing leadership. Demonstrates patient-focused and cost-effective approaches to care. Documents in an accurate and thorough manner in compliance with relevant standards. Facilitates patient throughput on a shift basis. Contributes to a supportive care environment for clinical colleagues and patients. Advances clinical competence in nursing practice.
