This includes intermittent skilled nursing services and directs, supervises, and instructs hospice aide staff in the provision of personal patient care. As an On-call Registered Nurse you will : \n\nKey Responsibilities : \n

• Assume primary responsibility for the patient / family caseload that includes assessing, planning, implementing, and evaluating care needs during on-call hours in collaboration with the triage team\n
• Report On-Call activities to the clinical leadership and Branch RN Case Managers at the end of the call shift\n
• Maintain and complete appropriate care coordination notes for visit notes, and other pertinent clinical information\n
• Assist, provide, or arrange for delivery of medications and supplies needed after business hours, weekends, and / or holidays\n
• As scheduled, attend interdisciplinary team meetings during on-call period for full team discussion, consultation and evaluation of patient / family needs\n
• Under the physician's order, admits patients eligible for hospice services in collaboration with the triage team\n
• Reports patient status and need for other disciplines to clinical leadership, attending physician and hospice physician\n
• Develops patient care plan that specifically addresses identified patient problems; nursing problems and goals that has been identified as mutually agreed upon goals with patient(s) during on-call hours, weekends and / or holiday(s)\n
• Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care\nSkills and Experience Required : \n
• Is currently licensed in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)\n
• Current driver's license and automobile in good working condition with proof of auto insurance\n
• Two years recent experience as a professional nurse\n
• Is able to meet certain physical demands of standing, bending, lifting, stooping or performing other work requiring light physical exertion (up to 30 pounds) on a continuous basis (over 50% of time); or moderate physical exertion (30 to 50 pounds) on a frequent basis (16 to 50% of time); or heavy physical exertion on an occasional basis (up to 15% of time)\n
• Ability to work in a field setting and exhibited ability to make sound nursing judgments; must be able to work independently\n
• Ability to assess patient needs and formulate individualized patient care plans to meet those needs\n
• Knowledge of physical, psychosocial and spiritual needs of terminally ill patients and their caregivers\n
• Effective written and verbal communication skills\nPreferred Knowledge, Skills and Experience : \n
• Two years' experience within hospice\n
• Certified Hospice and Palliative Nursing\n
• Electronic Medical Records Experience
