Licensed Master Social Worker (LMSW) – Hybrid Opportunity
Key Responsibilities
Conduct comprehensive assessments for patients referred by Primary Care Providers using evidence-based screening and assessment tools
Provide treatment for behavioral health conditions using evidence-based modalities, including:
Cognitive Behavioral Therapy (CBT)
Motivational Interviewing
Crisis Intervention
Interpersonal Psychotherapy
Behavioral Activation
Qualifications & Requirements
Active LMSW licensure
Experience assessing risk and collaborating with psychiatric and clinical teams on risk management
Excellent communication skills with the ability to convey clinical information clearly to integrated care teams, including PCPs, NPs/MDs, and Care Management staff
Bilingual Spanish required
