GENERAL STATEMENT OF RESPONSIBILITIES :

This is a FLSA non-exempt position in which the incumbent serves as a Case Manager at the Wellness Recovery Center, a Region Ten program for adults experiencing a mental health crisis. The incumbent is responsible for providing comprehensive case management and rehabilitative services to adults who are in crisis. This includes assessing service needs, developing treatment plans and providing assistance with access to community resources. He / she is expected to attend team meetings, other professional meetings and represent the program to the community as needed.

The incumbent reports to the Director III / Program Manager II of the Wellness Recovery Center and is expected to function with initiative and independent judgment, based on application of standard practices and with guidance from the supervisor. In carrying out position duties, he / she performs in accordance with applicable professional ethics and established Region Ten policies.

The Case Manager is part of the Wellness Recovery Center Clinical Group, which is responsible for the psychosocial rehabilitation and determination of course of treatment for the consumers.

MAJOR DUTIES : 1.

  • Responsible for admission and comprehensive orientation of consumers entering the program.
  • 2.

  • Gathers and communicates insurance information during admission process.
  • 3.

  • Completes weekly review of insurance status and consumer demographics.
  • 4.

  • Provides interventions in both individual and group settings within scope of education and ability.
  • 5.

  • Prepares and provides daily psycho-education groups with a focus on reduction on mental health symptoms, thinking errors, coping skills, emotional regulation, interpersonal relationships, distress tolerance, mindfulness, and re-stabilization.
  • 6.

  • Provides clinical case management to provide medical, psychiatric, therapeutic, occupational, transportation, housing, financial, legal, social, educational, and spiritual referrals to external agencies and providers.

  • Assists clients in developing their discharge plan with emphasis on follow-up appointments with community providers.
  • 8.

  • Provides consultation, information and support to individuals and families regarding the Wellness Recovery Center programming, services, and operations.
  • 9.

  • Answers phones and provides accurate information to callers and / or transfers to appropriate individuals or departments
  • 10.

  • Maintains comprehensive and up-to-date knowledge of area providers, services, and agencies relevant to population.
  • 11.

  • Monitors and ensures the health and safety of clients.
  • 12.

  • Monitors behavioral and psychiatric status of all clients on milieu.
  • 13.

  • Supports clients and maintains therapeutic relationship to maximize insight and management of symptoms and behavior.
  • 14.

  • Monitors client participation in program routines and community integration needs
  • 15.

  • Assists with self-administration of medications; observes, reports and documents side effects.
  • 16.

  • Maintains complete and current clinical records, service data and required documentation.
  • 17. Transports participants to and from the center as necessary.

    18. Acquires and maintains valid certification in First Aid, CPR, medication administration, non-aversive behavior management and defensive driving in order to ensure the safe and efficient operation of the program.

    19. Attends in-service training and staff development programs to enhance mental health services.

    20. Reads all agency communication.

    21. Performs crisis assessment, intervention, stabilization, and referral assistance to clients within the crisis continuum as needed.

    22. Performs other duties as assigned.

    23. Hours are variable.

    24. Provide coverage in an on-call capacity as needed.

    QUALIFICATIONS

    EDUCATION :

  • Bachelor's degree in a human services program is preferred.
  • EXPERIENCE :

  • Experience working with individuals with behavioral health needs for at least two years is preferred.
  • CERTIFICATION / LICENSE :

  • The incumbent is required to possess QMHP status in the Commonwealth of Virginia.
  • Driving Consumers : To ensure the safe and efficient operation of the program, a valid Virginia Driver's License plus an acceptable driving record as issued by the Department of Motor Vehicles are required. For business use of a personal car, a certificate of valid personal automobile insurance must be provided.
  • KSAS

    Knowledge of :

  • Crisis intervention, psychiatric and substance abuse emergencies.
  • The nature of serious mental illness and / or substance abuse, including clinical and developmental issues.
  • Typical actions and side effects of psychotropic medications.
  • Mental health recovery models of care.
  • Treatment modalities and intervention techniques, such as group therapy, behavior management, supportive counseling, family education, crisis intervention, discharge planning, and service coordination.
  • Consumer rights.
  • Services and systems available in the community including primary health care, support services, eligibility criteria, intake processes, and generic community resources.
  • Effective oral, written and interpersonal communication principles and techniques.
  • General principles of record documentation.
  • Medical issues related to mental illness; substance abuse symptomatology;
  • The service planning process and major components of a service plan.
  • Skills in :

  • interviewing;
  • observing, recording and reporting on an individual's functioning
  • identifying and documenting a consumer's needs for resources, services and other support;
  • understanding individual service plans;
  • using assessment tools to determining client's status, suicidal / homicidal behavior, and / or ability to care for self;
  • identifying services within the community and established service system to meet the individual's needs;
  • promoting consumer's attainment of treatment goals,
  • coordinating resources, services and activities in line with individualized service and discharge plan and follow up;
  • group and crisis counseling; and
  • Computerized documentation.
