Job Description
American Traveler is seeking an experienced RN Case Manager with a UT or compact RN license and AHA BLS certification for a hospital-based case management role with weekday hours.
Responsibilities
Position is based in a hospital case management departmentPatients include those requiring discharge planning and care coordination services8-hour day shifts, Monday through Friday, 8 : 00 am to 4 : 30 pm, with occasional weekend rotationNo on-call is requiredEpic is used for charting and documentationMay be required to float to other locations within a 60-mile radius or designated float zone, with duties possibly outside original skill setConduct admission reviews, patient assessments, and facilitate interdisciplinary discharge planningCoordinate and monitor use of resources, participate in performance improvement, and manage length of stayActively engage in utilization review, denial management, and work with third-party payersCollaborate with interdisciplinary teams, including social workers and homecare coordinators, for seamless patient transitionsDocument all case management actions in EpicParticipation in flu vaccination program is required during flu season; only medical exemptions with physician signature are allowedFirst-time travelers will be consideredRequirements
Active UT or compact RN license required with no pending statusAssociate Degree in Nursing required; Bachelor's Degree preferredMinimum of 1 year of nursing or case management experience requiredAmerican Heart Association Basic Life Support (AHA BLS) certification requiredFamiliarity with regulatory requirements and accreditation standards is preferredExperience with Epic EMR is preferredCandidates must reside more than 60 miles from the facility to qualify for travel statusBenefits
