Job Description

American Traveler is seeking an experienced RN Case Manager with a UT or compact RN license and AHA BLS certification for a hospital-based case management role with weekday hours.

Responsibilities

  • Position is based in a hospital case management department
  • Patients include those requiring discharge planning and care coordination services
  • 8-hour day shifts, Monday through Friday, 8 : 00 am to 4 : 30 pm, with occasional weekend rotation
  • No on-call is required
  • Epic is used for charting and documentation
  • May be required to float to other locations within a 60-mile radius or designated float zone, with duties possibly outside original skill set
  • Conduct admission reviews, patient assessments, and facilitate interdisciplinary discharge planning
  • Coordinate and monitor use of resources, participate in performance improvement, and manage length of stay
  • Actively engage in utilization review, denial management, and work with third-party payers
  • Collaborate with interdisciplinary teams, including social workers and homecare coordinators, for seamless patient transitions
  • Document all case management actions in Epic
  • Participation in flu vaccination program is required during flu season; only medical exemptions with physician signature are allowed
  • First-time travelers will be considered

    • Requirements

  • Active UT or compact RN license required with no pending status
  • Associate Degree in Nursing required; Bachelor's Degree preferred
  • Minimum of 1 year of nursing or case management experience required
  • American Heart Association Basic Life Support (AHA BLS) certification required
  • Familiarity with regulatory requirements and accreditation standards is preferred
  • Experience with Epic EMR is preferred
  • Candidates must reside more than 60 miles from the facility to qualify for travel status
