Source of Hope LLC is a dynamic and growing organization dedicated to making a positive impact in our community. We are seeking a highly motivated and organized Program Coordinator to join our team and help us achieve our mission.

Key Responsibilities :

  • Compliance
  • Oversee all reports & Quality
  • Approves providers' hours for Billing
  • Providers are training on their own onboarding
  • DDD external relation Communication

PCSP meeting

  • Program Growth

    • SOH Event

  • Policy adherence (DDD standards)
  • Review PCSP goals and add them to Spokechoice
  • Submit progress notes to DDD
  • Corrective actions (if needed)
  • Manage service expectations
  • Growth & improvement :

    • Looking for new members (management)

  • Process improvement
  • Staff and providers' support & coaching

    • Qualifications :

  • Bachelor's degree in a related field or equivalent work experience.
  • Fluent in both English and Spanish.
  • Proven experience in program coordination or a related role.
  • Strong organizational and project management skills.
  • Excellent communication and interpersonal abilities.
  • Proficiency in Microsoft Office, Excel, and project management tools.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and decision-making skills.
