- Responsibilities
- Conduct comprehensive assessments of patients' functional abilities and needs.
- Develop individualized treatment plans based on patient evaluations and goals.
- Implement therapeutic interventions to enhance patients' physical, emotional, and cognitive skills.
- Provide education to patients and their families regarding treatment plans and home exercises.
- Collaborate with interdisciplinary teams to ensure coordinated care and effective discharge planning.
- Maintain accurate documentation of patient progress and treatment outcomes.
- Participate in case management activities to optimize patient care pathways.
- Stay updated on best practices in occupational health and therapy techniques.
- Requirements
- A valid Occupational Therapy license in the state of practice.
- Experience in patient service with a focus on home health or skilled nursing facilities is preferred.
- Knowledge of discharge planning processes and case management principles.
- Familiarity with working with individuals with disabilities, autism, or other special needs is a plus.
- Strong communication skills to effectively interact with patients, families, and healthcare professionals.
- Ability to adapt treatment approaches based on individual patient needs in acute care settings or outpatient environments.
