About The Position

Job Title : Physical Therapist

Shift : Monday to Friday, Days

Hours per Shift : 32 hours guaranteed

Experience : New graduates are welcome. SNF experience preferred.

License : Valid state license required.

Certifications : Proof of recent Covid test required 72 hours prior to start date.

Must-Have :

• Completed prescreen form attached upon submission.
• Resume and license verification attached upon submission.
• References (3) provided.
• County criminal background check for 7 years for all counties lived and worked.
• Federal criminal background check for 7 years or more.
• Multi-state criminal search for 7 years or more.
• Nationwide sex offender search for 7 years or more.
• HHS / OIG search completed.
• GSA search reviewed.
• Social Security number trace completed.
• One-step PPD or Quantiferon test or proof of positive PPD and chest X-ray.
• SAM search completed.
• CORI background check completed.
• 8 hours of dementia training completed.

Description :

If these items are not completed and attached, the candidate will be declined.

Covid testing must be completed 72 hours prior to the start date, with results issued 48 hours prior.
