Job descriptionConduct UAS-NY assessments in-home or via telehealth. Review medical records and ensure accurate documentation. Educate consumers and coordinate care with providers. Manage your own appointments and maintain compliance. 1–2 years of experience as an RN in one-on-one in-home, community, skilled nursing facility, long term and geriatric care required RN license valid in New York BSN is required Familiarity with Uniform Assessment System (UAS)-NY is a plus Strong communication, time management, and Electronic Health Record (EHR) skills Reliable transportation for in-person visits
