Key Responsibilities as a Hospice RN / RN Case Manager :
Serve as the primary contact for patients and families, guiding them through their hospice experience.
Deliver skilled, hands-on nursing care, tailored to each patient's Plan of Care, in their home, assisted living, or other preferred settings.
Carry out initial and ongoing assessments to monitor and adjust care based on patient needs.
Collaborate with a multidisciplinary care team, including physicians, LPNs, CNAs, social workers, chaplains, and others, to create and refine personalized care plans.
Provide emotional, physical, and spiritual support to both patients and their loved ones, offering education, comfort, and guidance during bereavement.
Teach caregivers safe and confident care techniques, providing practical training and emotional support.
Communicate significant updates regarding patient conditions to the care team, ensuring appropriate and timely interventions.
Maintain accurate and current clinical documentation, adhering to infection control and compliance standards.
Support quality improvement and scheduling initiatives to ensure that care is consistent, compassionate, and efficient.
