EPIC experience is required.

Skilled in placement for SNF / IPR / LTAC / Hospice / Home Health, DME, Behavioral Health needs / DSS, and working with the homeless population.

Care management skills : assessment, care coordination, and discharge planning.

Description :

We are seeking a dedicated RN Case Manager.

The average census and case load is 99% capacity on most days, with 14-16 patients depending on the unit assigned.

Patient population varies, including adult medical / surgical, cardiac, and orthopedic patients.

Support within the department includes a director, managers, a chief medical officer, and a supportive utilization management team.

Peer case managers and social workers are available for collaboration.

Technology and equipment used includes an EMR system.

The department culture is very supportive and teamwork-oriented.

Managers communicate effectively regarding changes, and educators provide support throughout these changes.

Additional requirements include a willingness to work all holidays unless approved off in contract.

Shifts may vary based on the requisition, with an expectation of working weekends as needed.

Candidates must be adaptable and willing to float to different units as necessary.
