The Position :

St. Patrick Center is hiring a Housing Focused Case Manager. The case manager assists individuals exiting homelessness and entering housing with obtaining and retaining permanent housing and financial stability, engages clients by establishing trust and collaborative relationships, and provides outreach services to clients, among other duties. Moreover, the Housing Focused Case Manager will have prior experience working with socially and economically disadvantaged individuals and assisting with housing placement.
