• 2 locations : Prestonburg, KY (2 weeks) Hazard, KY (1 week)
• Patient volume : 15 ppd
• Requirements : BC in Neurology + KY License (required) and WV license (preferred)
• Start date : 90 days for credentialing.
• Relocation Assistance : Yes.
• Travel stipend : Negotiable
