We have an exciting opportunity to be a part of a new program within Community Based Services (CBS) - Integrated Community Services (ICS)! This position will primarily report to work on-site in Minneapolis, MN, but assignment based Telework may be available at supervisor's discretion based on program needs. Upon hire employeeswill be required to complete a Telework agreement. Also, this position does require 25% travel to programs throughout the State of Minnesota and to attend in-person meetings and trainings as needed.

The Registered Nurse Consultant assesses and diagnoses the needs of the individual, develops individualized nursing plans to meet those needs in coordination with the multi-disciplinary team, and coordinates and guides the provision of health care services, protocols and strategies for individuals supported in CBS sites.
