This position reports to a Licensed Professional and Agency Director(s).
Education and Experience :
Preventive professional" means, within the MHIDD / SAS system of care :
- An individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession, except a registered nurse who is licensed to practice in the State of South Carolina by the South Carolina Board of Nursing who also has four years of full-time accumulated experience in MHIDD / SA with the population served; or
- A graduate of a college or university with a Master's degree in a human service field and has one year of full-time, post-graduate degree accumulated MHIDD / SA experience with the population served, or a substance abuse professional who has one year of full-time, post graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or
- A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, post-bachelor's degree accumulated MHIDD / SA experience with the population served, or a substance abuse professional who has two years of full time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; or
- A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, post-bachelor's degree accumulated MHIDD / SA experience with the population served, or a substance abuse professional who has four years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.

Development, monitoring and revision of person-centered plans and plans of careCoordination of other services and supports related to maintaining an individual's health, safety, and well-being in the communityFacilitating person centered planning, team meetings,Facilitating the service delivery process, including initial assessment, ongoing monitoring of care and servicesMonitoring to assure quality care and continued appropriateness of servicesSubmitting person centered plans to Value Options or LME for authorizationEnsuring linkage to the most clinically appropriate and effective servicesSupportive counseling to address the diagnostic and clinical needs of the recipientAssistance with therapeutic interventionsPsychoeducation and training of family, unpaid caregivers,Relapse prevention and disease managementOngoing symptom monitoring and managementService coordination activities within the established person-centered planDocumenting service provision accurately and timelyAttending staff meetingsQuality improvement activitiesParticipation in continuing educationPerform NCI TrainingMarketingAssist the directorOther duties as assigned by supervisor or director
