Clinic hours include the following :

Monday – Thursday 8 : 00AM – 9 : 00PM

Friday 8 : 00AM – 8 : 00PM

Saturday 9 : 00AM – 5 : 00PM

SUMMARY

Family Service League is seeking a full-time Counselor for our Family Recovery Center (FRC) in Riverhead. The Counselor will provide individual and group therapy to children, families, and adults with the full range of DSM diagnoses in an integrated licensed clinic, providing both substance use, mental health, and primary care services.

CASAC-T / CASAC – $47,000 - $50,000

LMSW / LCSW / LMHC - $57,000 - $63,000

RESPONSIBILITIES

  • The Counselor will conduct intake assessments, formulate behavioral health diagnoses, and establish treatment recommendations in collaboration with the client after establishing an initial Phase of Treatment.
  • Provide trauma-informed, individual and group therapy to adults, adolescents, and children with the full range of DSM diagnoses, including both mental health and substance use disorders, in an acute care, outpatient setting.
  • Monitor progress towards goals with the outcome centralized on discharge planning after utilizing short-term, evidenced based interventions.
  • Complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record, including progress notes, intake assessments, care coordination, discharge planning, and treatment plans.
  • The Counselor will provide crisis interventions as needed.
  • Facilitate linkage to additional resources and provide follow up to clients during stabilization period and immediately following.
  • Coordinate with outside providers and collateral contacts to obtain collateral reports.
  • Coordinate treatment with a multidisciplinary team. Foster teamwork, work cooperatively and effectively with others to set goals, resolve problems, and make decisions.
  • The Counselor will complete training and maintain proficiency with evidence-based practices and crisis intervention techniques.
  • Attend routine supervisions and team meetings.
  • All other duties as assigned.
  • QUALIFICATIONS

    LMSW, LCSW, LMHC, CASAC, or Limited Permit holders are eligible to apply.

    At least 1 year experience in a behavioral health setting required; clinic experience preferred.

    Experience in applying trauma-informed care, person-centered service provision, and harm reduction strategies strongly preferred.

    Proficient computer skills, including Microsoft Office and Electronic Health Records required.

    Excellent interpersonal skills required.

    Strong verbal and written communication skills required.

    Ability to problem solve, make clear decisions and work as part of a team.

    Some evening and weekend hours may be required.

    Bilingual Spanish strongly preferred.

    PHYSICAL REQUIREMENTS

    This position requires sitting and standing for long periods of time.
