Job description

Job Summary :

The Registered Nurse will deliver skilled nursing care, case management, and hospice services to individuals facing age-related challenges, frailty, and serious illnesses. Key responsibilities include patient assessments, medication administration, treatment planning, and collaboration with an interdisciplinary team. Effective communication and strong collaboration with patients, providers, and team members are essential.

Location : Santa Monica, California, United States

Responsibilities :

  • Conduct patient assessments.
  • Administer medications.
  • Develop and implement treatment plans.
  • Collaborate with the interdisciplinary team.
  • Provide skilled nursing care.
  • Manage patient cases.
  • Deliver hospice care.
  • Communicate effectively with patients, providers, and team members.

Required Skills & Certifications :

  • Clinical Assessment skills
  • Medication Administration skills
  • Treatment Planning skills
  • Active Registered Nurse license in the state of working
  • Bachelor's degree in Nursing or related field
  • Driver's license, car insurance, and automobile for field work
  • BLS certification
  • ARRT certification
  • CRT certification
  • Minimum 1 year of experience

    • Preferred Skills & Certifications :

  • Two years minimum of hospital, home health, or hospice care experience
  • Interdisciplinary Team Collaboration skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment

    • Special Considerations :

  • Weekend availability is required.
  • Every other Saturday rotation from 8 : 00 AM to 12 : 00 PM.
  • No pending licenses accepted.
  • RTO (request time off) must be noted at the time of submittal.

    • Scheduling :

  • Start date : ASAP
  • Every other Saturday rotation from 8 : 00 AM to 12 : 00 PM.
