Signify Health, part of CVS Health®, is hiring full-time physicians (MD / DO) to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

In this role, you will :

• Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area (majority of visits scheduled weekdays between 8 a.m. and 7 p.m.)
• Primary coverage areas include but are not limited to : Fairfield, New Haven counties
• Provide 40 hours of availability weekly, consistently providing 160+ capacity units per month
• Execute a strong visit completion rate, consistently pacing 110+ completed visits per month
• Execute a collaborating physician agreement, partnering with and overseeing Signify Health-contracted nurse practitioners or physician assistants (and fulfilling all state-by-state requirements)

Other role expectations and requirements :

During our In-Home Health Evaluations (and other associated follow-up visits), you'll meet people where they are and re-engage them with their health care during an approximately 45-minute pre-scheduled visit.

The visit includes a medication and medical history review, a physical evaluation, and, if ordered by the person's health plan, additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.). You will be provided a pre-populated, patient-specific form for each evaluation and are not required to prescribe medicine, order lab tests, or alter people's current treatment regimen.

This pay range represents the base pay for each In-Home Health Evaluation (and associated visit) completed by a physician. The number of In-Home Health Evaluations (and associated visits) completed per day may vary. Pay is dependent on a variety of factors including experience, education, geography and other relevant criteria. This position is eligible for other reimbursement opportunities, including pay per completed Diagnostic and Preventive Service (DPS), as ordered, in addition to the base pay listed above. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
