Social Worker Qualifications

Active LSW (Licensed Social Worker) or MSW (Master of Social Work) credential

Minimum 1 year of experience as a Social Worker in a healthcare or care management setting

Experience in long-term care, corrections, case management, or emergency room social work preferred

Ability to work collaboratively in a multidisciplinary healthcare team

Strong communication, assessment, and organizational skills

Compliance with onboarding requirements, including health screenings and background checks

Social Worker Responsibilities

Deliver comprehensive social work services to patients and families in the care management unit

Conduct psychosocial assessments and create individualized care plans

Coordinate discharge planning and connect patients with Maryland community resources

Advocate for patient needs and support transitions of care

Participate in interdisciplinary team meetings with healthcare professionals

Maintain accurate and timely documentation in accordance with hospital policies

Participate in weekend rotation as scheduled
