Job description
Performs pharmacotherapeutic monitoring and provision of pharmaceutical care for oncology patients from initial education through the entire pharmaceutical care of the patient
Supervises and provides oncology pharmacy clinical activities, including but not limited to, antineoplastic, cancer pain, and general oncology pharmaceutical care for patients in our Cancer Center and inpatient care areas
Supervises and provides activities within the medication preparation, dispensing, and administration processes for Cancer Center, Outpatient Infusion Clinic, or other patients
Assists in the preparation and maintenance of patient care plans and automated clinical care systems
Provides general and patient specific drug information to patients, physicians, nurses, and other health care professionals
Interacts routinely and works collaboratively with physicians, nurses, and other health care professionals to provide optimal pharmaceutical care to patients
Identifies, resolves, and prevents potential and actual medication-related problems including untreated indications, improper drug selection, sub-therapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without indication
Initiates, adjust, monitor, and recommend changes in drug therapy based on information obtained through collaborative practice, patient interviews, objective monitoring parameters, review of drug profiles and the patient medical record
Coordinates and interpret laboratory monitoring of drug therapy, including drug dosing to established protocols and best practice, serum drug levels, electrolytes, hematology, metabolic parameters, and genetic biomarkers along with other drug therapy-related laboratory testing
Provides patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization and patient compliance, detecting and / or minimizing adverse drug reactions, eliminating unnecessary drug use, and duplication Supports clinical pharmacists in providing pharmaceutical care to patients by direct interaction, and formal educational programs
Maintains expertise in areas of practice through continual learning, journal review and other educational activities
Educates pharmacists, technicians, pharmacy students, post graduate residents, allied health professionals, nurses, and physicians in pharmacotherapy within specialty area knowledge and skills
Focuses pharmacy clinical activities to decrease overall patient healthcare costs by effective pharmacotherapy, which results in better patient outcomes and efficient use of Cancer Center and other organizational resources
Provides input and drug information to the Pharmacy & Therapeutics Committee to support efficacious and cost-effective oncology drug therapy
Maintains interface with medical staff to provide information pertaining to new pharmaceuticals and therapies, formulary activities, drug usage, new protocols, and best practice regarding clinical drug use
Supports and participates in research study activities in specialty areas of knowledge and expertise
Assists in supervision of the preparation, labeling and dispensing of drugs and other pharmaceutical supplies; ensures compliance with federal, state, and local laws and regulations pertaining to the dispensing of drugs and narcotics and maintenance of all required records
Supervises and participates in data gathering, reporting, analysis, and quality improvement activities to support pharmacy oncology and infusion services
Participates in activities to expand pharmacy oncology and infusion services
