• Monday - Friday, 8am - 5pm schedule with hybrid work model
• 12 - 15 patients per day
• Clinic-based setting with home health visits
• Split schedule : 3 days field work, 2 days telehealth
• Geriatric patient population focus
• Hybrid role combining telehealth and home health visits
• Three years adult primary care experience required
• Must be comfortable working independently
• We provide complimentary housing and travel
• We arrange and cover costs for licensing and malpractice
• We simplify the credentialing and privileging process
