Case Manager
Overview
The Case Manager plays a key role in supporting the clinical team at Cedar Recovery by facilitating seamless internal and external case management for patients navigating substance use and behavioral health treatment. This individual collaborates closely with medical providers, therapists, and peer support specialists to ensure patients receive integrated, person-centered care aligned with their recovery goals.
The ideal candidate will demonstrate emotional intelligence, strong organizational skills, and an ability to foster trust-based relationships with patients and care team members. This position focuses on external care coordination while supporting internal referrals and follow-ups when necessary.
Responsibilities

- Serve as a central point of contact for coordinating services across internal teams (medical, therapy, peer support) to promote continuity of care.
- Track patient progress across different services to support recovery milestones, treatment plan goals, and quality metrics.
- Assist with warm handoffs between departments (e.g., from provider to therapist or peer support).
- Identify and address barriers to care (e.g., appointment scheduling conflicts, transportation needs, communication gaps).
- Maintain up-to-date documentation in the EMR related to care coordination activities.
- Coordinate external referrals to primary care, specialty care, housing, or other community-based services as needed.
- Support the patient in navigating transitions between levels of care (e.g., detox, inpatient, outpatient).
- Educate patients on Cedar Recovery’s program structure, policies, and the role of each care team member.
- Build rapport with patients and support long-term engagement through relational, strengths-based care.
Qualifications
Minimum Education Requirement :
Bachelor’s degree in one of the following (or closely related) fields :Social Work
PsychologyHealth and Human ServicesPublic HealthCounselingPreferred Qualifications :
Experience in behavioral health, addiction treatment, or medical settingsFamiliarity with EMR systems and care team documentationKnowledge of social determinants of health and how they impact recoveryBilingual or bicultural candidates encouraged to applyWork Environment & Hours
This job operates in a professional treatment environment with a mix of in-person and virtual care team collaboration.This is a full-time, 40-hour-per-week position, Monday through Friday from 9 : 00 a.m. to 5 : 00 p.m.May include participation in a rotating after-hours call schedule approximately once per quarter.Eligible employees may request a hybrid schedule based on team needs.Compensation and Benefits
