Coverage area : 60047, 60010, 60021, 60050, 60051, 60047

Patients typically have multiple comorbidities and can include (but not limited to) : CHF, COPD, pneumonia, cancer, dementia, kidney disease, hypertension, fall, fracture, infections, cellulitis, post-surgical, wound care, ostomy care, labs, IV infusion, etc. EPIC EMR charting, Sapphire IVs. Will be given ADVOCATE laptop and phone. Any solid color scrub is acceptable. Traveler must have own transportation for all patient visits.
