Job Summary and Qualifications

The Nurse Navigator acts as a consultant, facilitator and educator to the Imaging service line patient population. Responsibilities include assisting patients through the Imaging care delivery system through patient education, coordination of care, patient advocacy and communication with the multi-disciplinary team as well as the patient and family as appropriate. The navigator will maintain communication with referring providers and be comfortable with discussing best practice guidelines and standard of care. Navigation responsibilities include maintaining an ongoing database on patients and be comfortable generating reports related to program metrics.
