Clinical Coordinator – Rehabilitation Center\nFull-Time | Monday–Friday 7a-3 : 30p | 1 Weekend rotation a month \n\nPosition Summary : \nThe Clinical Coordinator plays a key leadership role within the rehabilitation team, supporting nursing staff and ensuring high-quality, resident-centered care. This position focuses on improving clinical outcomes by coordinating care plans, collaborating with providers, and guiding daily clinical operations.\n\nPay : \nCompetitive and based on clinical and leadership experience. Additional compensation provided for hours worked after 3 : 00 PM and on weekends.\n\nResponsibilities : \nCoordinate clinical services for all residents in the rehab unit\n\nOversee individualized care plans and ensure follow-through by nursing staff\n\nAssign staff based on care needs and ensure effective communication\n\nProvide emotional and social support to residents and families\n\nAssist nurses with bedside care and participate in audits and floor rounds\n\nFoster a culture of compassion, responsiveness, and teamwork\n\nQualifications : \nActive NYS RN or LPN license required\n\nMinimum 1 year of experience in long-term or acute care\n\nAt least 1 year of formal healthcare leadership experience\n\nRehabilitation experience strongly preferred\n\nCurrent CPR certification required\n\nStrong communication and leadership skills\n\nMust meet NYS Department of Health immunization requirements\n\n
