Job Summary
Job Summary : Job incumbent supports and serves an advocate for formerly chronically homeless substance abusing adults residing at the TCC Housing First Facility. Assists residents with financial management, activities of daily living, medical and behavioral health appointments and serves as a liaison with other providers and agencies.
Adhere to the TCC Ch'eghwtsen' model of service and guiding principle which requires providing timely and effective service along with the ability to interact with others in a way that inspires trust and demonstrates respect, compassion and empathy.
Essential Functions

Representative Duties : Under the general supervision of the Housing First Manager and Housing First Clinician, job incumbent will :
1. Assist residents in obtaining financial support to insure basic needs are met.
2. Assist residents in scheduling activities of daily living, including shopping, recreation, social interactions and vocational skill building.
3. Assist residents in keeping appointments with behavioral health and health providers.
4. Provide functional assessments, BH Intakes and ROI, CSR, alcohol assessments, individual and group skills development, provide case management and comprehensive recovery support services to assist residents with behavioral health, social skills, recreational, self-maintenance and daily living.
5. Maintain clinical and administrative records as needed according to policy and to Medicaid standards and submits reports and data in an accurate and timely manner.
6. Gather and prepare information to meet all grant reporting requirements. Maintains data collection for grant reports through the AKHIMS and Service Point systems. Assists HF Director with grant reports, as needed, with such data.
7. Coordinate with other private and public human services providers.
8. Provide crisis intervention as needed to residents.
9. Refers residents for specialized services within or outside the agency.
10. Routinely inspects the physical facility to insure all systems are operable and in good repair and reports deficiencies to maintenance staff in timely manner.
11. Maintain regular contact with residents to insure the safety of residents, building and grounds.
12. Promote a positive living environment by circulating throughout the building and encouraging interactions with the residents and among residents by hosting weekly informal gatherings such as; game night, barbecues, movie night, etc. and advertising these activities each week in advance.
13. Ensure information in resident files is current and correct.
14. Help residents establish and maintain appropriate relationships with neighbors, fellow residents and Housing First staff.
15. Assist residents in maintaining permanent housing through the use of motivational techniques, and coordination of necessary support services, treatment options, and rental education classes.
16. Participates in Housing First team meetings.
17. As directed, will oversee assigned residential support staff team and coordinate work, train and assign work to team members.

Minimum Qualifications
Minimum Qualifications :
1. At least three years of experience providing services to adults with chronic behavioral health and substance abuse issues, preferably as a case manager.
2. Bachelor's degree in Human Service or Social Work preferred or equivalent experience may be substituted.
3. Three years' experience providing behavioral health or case management services required.
4. Must have valid driver's license and clean driving record.
5. Must have CDC1 or CDCII certification.
6. Must maintain strict client confidentiality.
7. Must pass background check pursuant to the State of Alaska background check and Federal Indian Child Protection and Family Violence Prevention Act requirements.
Knowledge, Skills and Abilities :
1. Knowledge of Medicaid third party billing required.
2. Cross-cultural treatment skills and knowledge of Alaska Native cultures and traditions required.
3. An understanding of and commitment to the Housing First philosophy.
4. Strong computer skills using electronic health records and Microsoft Office applications.
5. Strong verbal communication, analytical and interpersonal skills required.
6. Must have initiative and prioritizing skills.
7. Must work as part of a team.
8. Must have excellent customer service skills and the ability to build rapport with clients, while respecting privacy and confidentiality.
9. Knowledge and ability to adhere to Health Insurance Portability and Accountability Act (HIPAA) and 42 CFR Part 2 confidentiality regulations.
10. Must demonstrate ethical commitment to mental wellness, recovery and goals of treatment within Tanana Chiefs Conference Health Services.
Supervision : This position has no supervisory responsibilities.
Supervision
Physical demands : Must be able to lift and carry up to 25 pounds. Must be able to lift and support 50-100 pounds to help lift or support a client. Hours worked may be day, evening, nights or weekends. Must be able to work efficiently and effectively in a busy and sometimes stressful environment.
