Job Description :
The Family Health Center of Worcester (FHCW) School-Based Health Centers (SBHCs) are seeking a Family or Pediatric Nurse Practitioner to join our team. The Nurse Practitioner provides comprehensive, cost-effective, and high-quality outpatient health care to children, adolescents, and their families in a school-based setting. This role includes delivering diagnostic, treatment, and preventive services; managing chronic illnesses; and promoting health education for students and their families. Care is provided in alignment with the standards of advanced nursing practice and the mission of FHCW to improve access to equitable health care for all.
The Family or Pediatric Nurse Practitioner or the Physician Assistant works as part of a multidisciplinary care team that includes a Community Health Worker and Behavioral Health Clinician, collaborating closely with school staff to support the overall health and well-being of students. The Family or Pediatric Nurse Practitioner or the Physician Assistant also serves as a vital liaison between the health center and the broader school community.
Work Location and Schedule : This position is based at Sullivan Middle School, 140 Apricot Street, Worcester, MA, within the Worcester Public Schools. Regular hours are Monday through Friday, 7 : 00 a.m. to 3 : 30 p.m., with occasional coverage at the Family Health Center of Worcester’s main site at 26 Queen Street. School-Based Health Center (SBHC) assignments may vary based on program and patient care needs. Any changes in site placement will be communicated with reasonable notice by the SBHC Director.
Duties and Responsibilities :
1. Provide comprehensive primary care to Worcester Public School students and their families, and other FHCW pediatric patients as needed, including well-child exams, acute and chronic care management, and preventive services.
2. Deliver patient- and family-centered health education to promote wellness, prevent illness, and support ongoing disease management.
3. Collaborate with the School-Based Health Center team, medical leadership, and multidisciplinary team members to ensure high-quality, coordinated care.
4. Partner with school staff and district personnel to identify student health needs and align clinic services with school and community priorities.
5. Coordinate care and referrals with community providers, specialists, and social service organizations to support holistic care.
6. Participate in quality improvement activities, including chart audits, program evaluation, and outcome tracking.
7. Participate in staff and provider meetings, contributing to decision-making and problem-solving related to school-based health services.
8. Assist in developing and achieving program goals, objectives, and quality improvement initiatives.
9. Oversee vaccine management in accordance with MDPH, CDC, and FHCW protocols, including proper storage, handling, documentation, and reporting.
10. Maintain current licensure, certification, and continuing education requirements consistent with the scope of practice.
11. Provide periodic on-call coverage (approximately 4–6 weeks per year) to address urgent patient needs.
Qualifications :
1. Minimum of two years of relevant experience as a Nurse Practitioner required. Experience in Pediatrics strongly preferred.
2. Commitment to providing equitable, culturally responsive, trauma-informed care.
3. Strong interpersonal and communication skills with the ability to engage effectively with youth, families, school staff, and multidisciplinary teams.
4. Ability to work independently in a school-based environment while maintaining a strong connection with Team Lead and SBHC colleagues.
5. Proficiency in electronic health records (EHR) and documentation standards.
6. Flexibility in meeting the needs of a dynamic school health program.
Educational Requirements :
Graduate of an ANA-accredited Nurse Practitioner program. Master’s Degree in Nursing required.
Professional Licensure / Special Skills and Certification Required :
Current licensure by the Massachusetts Board of Nursing
Nationally certified as a Family Nurse Practitioner, Pediatric Nurse Practitioner, or School Health Nurse Practitioner
Current DEA / Mass Control Substance license for prescriptive authority.
CPR / BLS required.
Reporting Relationship : Nurse Practitioner Team Lead, Director of School-Based Centers
