Case Management Supervisor - Full Time

Clinical / Allied Health

Under the direction of the Manager of Case Management, supervises functions of case management including basic discharge planning activities to achieve desired outcomes. Participates in the establishment of policies, procedures and work processes. Maintains performance improvement activities within the department and participates in the department services first goals and objectives.

Education / Experience Required :

• Bachelor's degree in nursing, healthcare management or related health care field required or a Masters of Social Work required.
• Minimum of three (3) years clinical experience; hospital-based case management preferred.
