Position Summary
The Scheduler / Intake Coordinator is responsible for receiving new patient referrals, verifying insurance, and coordinating schedules for field staff. This role requires strong communication skills, attention to detail, calmness under pressure, and the ability to multitask in a fast-moving environment.
Responsibilities
- Answer incoming calls from patients, families, referral sources, and field staff.
- Communicate schedule updates and follow-ups clearly and professionally.
- Handle non-clinical patient questions and route clinical questions appropriately.
- Assign appropriate field staff based on skill, location, and availability.
- Manage schedule changes, replacements, call-offs, and urgent coverage needs.
- Maintain staffing patterns and time-off calendars for field staff.
- Document all scheduling-related communication in the EMR.
Intake & Documentation
- Receive new referral information and create patient records.
- Verify insurance coverage and eligibility.
- Assist with coordinating new patient admissions.
- Maintain accurate records and case management notes.
Support
- Assist with onboarding and transferring field staff between cases.
- Collaborate with nursing and administrative teams to ensure continuity of care.
- Perform other administrative duties as assigned.
Qualifications
- High School Diploma or GED required.
- Bilingual Spanish (required).
- 1+ year of scheduling or office coordination experience in Home Health..
- Familiarity with WellSky/Kinnser (or willingness to learn quickly).
- Strong computer proficiency (Word, Outlook, EMR system navigation).
- Excellent organization, multi-tasking, and follow-through skills.
- Must demonstrate professional communication and positive attitude under pressure.
- Ability to maintain confidentiality and adhere to home care regulations.
- Must have reliable transportation.
