Care Navigation Specialist, IOP

A company is looking for a Care Navigation Specialist, IOP.

Key Responsibilities

Organize and prioritize referrals for IOP and higher levels of care

Support members during transitions between different levels of behavioral health care

Collaborate with clinical staff to ensure care transitions align with treatment recommendations

Required Qualifications

2+ years' experience in a healthcare setting, preferably in behavioral health or IOP / PHP / inpatient settings

Direct experience working with high-risk mental health populations, including co-occurring substance use disorders

Strong understanding of the behavioral health continuum of care

Demonstrated success in patient-facing roles involving intake, enrollment, or care coordination

BA / BS degree preferred
