Multi-State Licensed Practical Nurse

A company is looking for a Remote Bilingual Licensed Practical Nurse (LPN).

Key Responsibilities :

Manage patient census to close gaps in clinical and non-clinical care

Identify and coordinate community resources for patient care

Provide education on chronic condition management and perform medication management

Required Qualifications and Education :

Active Multi-State / Compact License (LPN) (NCL) (LVN)

At least 3 years of direct patient-facing work experience

Technical aptitude with Microsoft Office Suite

Bilingual in Spanish and English

Experience with Epic is a plus
