Position Overview :
We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to provide high-quality nursing care during the night shift. The nurse will administer medications and treatments, monitor resident health status, supervise nursing assistants, and ensure all care provided aligns with established policies, procedures, and state regulations.
Key Responsibilities :
Administer medications and treatments safely, accurately, and in accordance with all applicable laws, policies, and procedures.Document all care, medications, and treatments accurately and in a timely manner.Communicate resident care information-both written and verbal-to the interdisciplinary team and other healthcare providers to ensure continuity of care, following all HIPAA regulations.Perform resident assessments and prepare reports, including incident and infection control reports.Participate in Quality Assurance and Performance Improvement (QAPI) projects as requested to improve resident outcomes.Supervise and support nursing assistants during assigned shifts to ensure quality and consistency of care.Resolve conflicts, address care-related issues, and provide on-the-job training and reinforcement as needed.Advocate for residents by ensuring care is delivered respectfully and in accordance with their rights, wishes, and directives.Maintain a professional and compassionate demeanor when interacting with residents, families, co-workers, and visitors.Report and document assessments, changes in condition, and findings to the physician as indicated.Initiate, review, and update resident care plans as needed.Ensure all clinical documentation is complete, accurate, and compliant with regulatory standards.Perform other duties as assigned to support facility operations and quality of care.Qualifications :
