Role Description
The Center Medical Director leads the clinical team at their center, ensuring best patient care and a great workplace to keep patients happy, healthy, and out of the hospital. The CMD practices primary care 80% of the time and spends 20% on administrative duties. Leadership responsibilities include modeling thoughtful primary care, mentoring and coaching providers, and engaging in organization-wide strategic initiatives.

- Drive implementation of OSH strategic initiatives.
- Follow direction from regional leadership (regional medical director or senior medical director) in collaboration with CMDs to implement regional priorities and reach regional objectives.
- Cascade organizational information to care team members and elevate care team concerns and ideas to the organization.
Provider Management
Coordinate and lead onboarding of new providers at the center.Model excellent medical care, implementation of the care model, and success in metrics.Ensure care teams prepare for and execute daily huddles, patient care planning and management, admission prevention work, and clinical documentation workflows.Ensure medical care is appropriate evidence‑based, keeping patients healthy and out of the hospital.Teach and implement Oak Street's clinical guidelines at provider and center levels.Support teams to provide highest‑quality primary and secondary preventative care.Educate and support providers in implementing the value‑based full‑risk model of care.Assess and address root causes when center metric goals are not reached.Leadership and Coaching
Create and maintain a culture of coaching and growth for providers and care teams.Listen to feedback and address concerns with constructive problem‑solving.Conduct performance reviews, discuss provider goals, and help develop plans to achieve them.Culture

Ideal Candidates Will Be
M.D., D.O., A.P.N., or P.A. graduates.Board certification in Internal Medicine or Family Medicine for M.D. / D.O., or family, adult, or gerontology for A.P.N.Experience in clinical leadership roles coaching providers.Experience using a metrics‑driven approach to medical care and / or quality projects.Passionate about work and motivated to join a fast‑growing organization.Self‑aware leadership skills and eagerness to share impact in a fast‑growing team.Commitment to older adults and low‑income patients.Comfort with ambiguity and love for problem solving.Calm under pressure, guiding others to smart solutions.5+ years of experience in outpatient practice (preferred).Fellowship training in geriatrics, other professional degrees (M.B.A., J.D., M.P.H.), and prior executive roles welcomed but not required.US work authorization.Position Details
