    • Requirements

  • Experience : Minimum 2 years of acute care hospital Case Management experience

    • Must include care coordination and discharge planning

  • Behavioral health-only experience does not qualify
  • Travel Experience : Required
  • Charting : Epic required
  • License : CA RN (pending accepted if active by start)
  • Certifications : Standard RN license; case management certifications preferred

    • Skills

  • Care Coordination & Discharge Planning
  • Plan of Care Management
  • InterQual Criteria (preferred)
  • CMS, DRG knowledge (preferred)
  • Familiarity with HMO / IPA systems
  • DME / SNF placement and ambulance coordination
  • Experience with interdisciplinary collaboration (social services, transport, etc.)
  • Proficiency in Microsoft applications
