Weekday Call : Mon-Fri, 4pm 7 : 30am

Weekend Call : Sat 7 : 30am Sun 7 : 30am and / or Sun 7 : 30am Mon 7 : 30am

• Call Type : Pager or Mobile Phone (Physician to carry pager or cell phone at all times while on-call)
• Call Ratio : Urologist shall provide weeknight call an average of 10 days per month, and weekend call coverage an average of one (1) weekend per month.
• Call Response Time : On-call providers must be available within 15 minutes by phone and on-site within 60 minutes.

Requirements :

• Active unrestricted license in any State or U.S. territory
• BC / BE by the American Board of Urology (ABU)
• BLS
• ACLS
• DEA Certificate
• NPI

Please note : Physicians who have ever had a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract.
