Primary Care Physician – Value-Based Outpatient Care

Compensation :

Base Salary : $215,000 – $245,000 (commensurate with experience and location)

Incentive Bonus : Based on quality metrics and patient outcomes

Sign-On Bonus & Relocation Package : Available

About the Role

We are seeking an experienced and compassionate Primary Care Physician (PCP) to join our expanding network of innovative neighborhood clinics. Our model emphasizes value-based, patient-centered healthcare—focusing on prevention, wellness, and better outcomes for local Medicare populations.

Our physicians are supported by large, integrated care teams, enabling them to deliver quality care over quantity of visits. You’ll practice medicine the way it’s meant to be—personal, proactive, and rewarding.

Role Details

Support Team : Scribe, Medical Assistant, and Social Worker

On-Site Resources : In-house lab with phlebotomist and access to RubiconMD for specialist consultations

Model : Value-Based Primary Care

Patient Transportation : Complimentary van pickup within 5 miles of the clinic

Community Engagement : Wellness activities including yoga, exercise, dance, and health events

Clinic Composition : Average of 3–5 providers per location (mix of Physicians and NPs)
